How to File a Death Claim
The Lafayette Life sales and service philosophy is based on
personalized service. We contract experienced professional insurance
agents and brokers to represent the Lafayette Life Insurance Company
to assist you in all service issues including filing claims.
If you have had a relative recently pass away and require assistance
filing an insurance claim please review the information
below.
Individual Filing
- Fill out a W-9 form: Include Name, Address, & Social
Security Number of the Beneficiary. (See
your Agent.)
- Fill out Beneficiary's Statement and Payment Direction form.
(See your Agent.)
- Obtain a certified copy of the Certificate of Death from the
Health Department.
(There are some special cases where more work will be
required. i.e. Trusts, Estates, Predeceased Beneficiary, and
Special arrangements)

sufficient information to contact you. Rest assured, your contact information
