At The Lafayette Life Insurance Company (“Lafayette Life”), we realize that the basis of any relationship is trust. As a result, we have put in place policies and practices that respect the privacy, not just of our customers, but of all individuals who use our company. We keep personal information about you private, but we may share information about you:
· with our affiliated companies;
· with third parties with whom we do business; and
· as otherwise permitted by law.
Due to the rapid changes occurring in the financial services industry, we reserve the right to revise our privacy policy as needed.
Security of Your Information
We have put in place procedures and policy guidelines to safeguard the privacy of your nonpublic personal information from unauthorized use or improper access. These security methods include physical, electronic and procedural safeguards. We will continue to monitor and review our security procedures as new technologies become available.
Employee Access to Information
We restrict access to data about you. Our employees are governed by a code of conduct. They are reminded of their obligations with regard to the privacy of customer information. An employee who violates our privacy policies is subject to discipline.
The Types of Information About You that We Collect
In the ordinary course of business, we collect, retain and use data about you:

· to administer your policy(ies) with us;
· to process and settle claims; and
· to keep you informed of products and services that may be of interest to you.
This data, known as nonpublic personal information, is from several sources and includes:
· data we receive from you on applications or other forms (e.g., name, address, date of birth, social security number, gender, marital status, health history, assets, and income);
· data about your transactions with us, our affiliates, nonaffiliated third parties and others (e.g., medical information, motor vehicle reports, and demographic reports); and
· data we receive from consumer reporting agencies (e.g., credit report and credit history).
We do not sell your information to third parties. Under no circumstances do we share medical information for marketing purposes with nonaffiliated companies.
Our Information Sharing Practices with Nonaffiliated Third Parties
We restrict the types of information about you that we share and the types of entities with whom we share it. The primary reason for sharing information about you is to provide you with more convenience in transactions with us and to give you more choices about insurance products.
Sharing Information with Lafayette Life’s Insurance Affiliates
We may share information with our affiliates to enable them to provide customer service, including claims services or account maintenance. Sharing your information with our affiliates enables us to serve you more efficiently. It also makes it more convenient for you to do business with us. We may also share other types of information with our affiliates such as information we receive from you on applications or other forms. We may also share information we receive from a consumer reporting agency or other third party.
Sharing Information With Service Providers or Joint Marketers
Information about you may be shared with a company that performs marketing services or administrative services on our behalf or to other financial institutions with whom we have a joint-marketing agreement. A service provider and others with whom we have a joint-marketing agreement or administrative agreement are obligated to protect your personal information through their own established policies concerning the privacy of your information. We may also require that these entities adhere to our privacy standards. These companies may use and disclose the information provided to them by us only for the purposes for which it is provided or to the extent otherwise permitted by law.
Sharing Information With Other Third Parties
We may disclose nonpublic personal information about you to nonaffiliated third parties as permitted or required by law. For example, we might be required to disclose information about you:
· in response to a subpoena;
· in response to a court order;
· for fraud prevention purposes; or
· to comply with an inquiry from a government agency or regulator.
Our Treatment of Information About Former Customers
Our protection of your nonpublic personal information extends beyond the period of your customer relationship with us. If you end your customer relationship with us or we end our customer relationship with you, we will not disclose your information other than as permitted by law.

Our privacy commitment extends to those companies with which we affiliate during the next 12 months from the date of our mailing of this notice to you. We will reaffirm this policy annually in writing, as long as you maintain a relationship with Lafayette Life.
Upon written request, we will make information from your file available for your review. If you notify us that any information is incorrect, we will review it. If we agree, we will correct your records. If we do not agree, you may submit a short statement of dispute. This statement will be included in any future disclosure of information.
This notice of our privacy policy is being sent to you in compliance with the Gramm-Leach-Bliley (“GLB”) Act. Any provision of this notice which may conflict with GLB, as amended, is hereby amended to conform to the minimum requirements of GLB.
